Home Visit Information Sheet

 

We are excited to see you in your home to assist with your mobility and wellness goals.

 

1. The therapists do their best to arrive on time. They may arrive 5-10 minutes before or after the scheduled time. To maximize your treatment time it is best to be dressed in exercise or loose fitting clothing and ready for the appointment about 10 minutes prior to the appointment time.

 

2. If you have a list of medications or medical reports you can email them to the clinic or have a copy for the therapist available. Please ensure your intake form is also completed prior to the appointment.

 

3. An invoice for the treatment will be sent after the appointment within 2 business day via email through the payment system Square. You can pay online by clicking the PAYNOW button. After payment is received an official receipt will be sent that can be used for insurance, tax and record keeping purposes. (E-transfer is also an accepted payment.)

 

4. Home program ideas and exercises will be sent to you via email within 24 hours of your appointment, unless otherwise discussed with your therapist.

 

5. We do our best to accommodate your schedule when booking home visit appointments. Our mobile therapist’s schedules fill up quickly and we may not be ableto provide your optimal time slot. We do our best to work with you!

 

Physiotherapy and Occupational Therapy Home Visit Fees

Initial assessment (60 minutes): $128

45 minute follow-up: $118

30 minute follow-up: $104

Late cancelation fee (less than 24 hours) $35
